Mission Statement
Our
mission is to offer support to individuals living with developmental
disabilities and mental illness. We seek to help individuals meet their
basic, psychological, physical, and social needs. We strive to preserve
human dignity while encouraging interdependence as we promote the quality of
life of our fellow man to higher standards.

Collaborating Concepts, Inc. offers the following services:

Home & Community Support We provide individuals with the support and encouragement necessary to
engage in positive and productive daily activities in their home and in
their prospective communities. Our professional support staff utilizes a
person-centered approach in educating and promoting a higher quality of life
for our consumers and their families.

Concepts Applied
Under Supported Employment (CAUSE)
Individuals will receive support in obtaining and maintaining gainful
employment. We provide assessments of the individual’s desires and
abilities through a person centered plan of care. We provide job
development to making the right job match for the individual, job coaching
to transition both employee and employer, and continued long-term support
for success on the job.

Diagnostic Assessments
We offer an intensive clinical and functional evaluation of a recipient’s
developmental disability that results in the issuance of a
Diagnostic/Assessment report with a recommendation regarding whether the
recipient meets target population criteria, and includes recommendations for
service delivery that provides the foundation for a person centered plan.

It is our philosophy that people have the capacity to improve their quality
of life with the sincere support of the professional. Our professionals
realize that each person is a multifaceted individual and provides services
in a manner that addresses every aspect of the person. Identified services
are person centered; requiring that the individual is completely involved in
the treatment planning process which establishes a team approach between the
professional and the person served.
**Outpatient
Therapy (Individual, Family, and Group)—Professionals
teach new skills or ways to cope with problems.
**We
provide referrals for Outpatient Therapy (OPT)
